OUR VISION

Every person with Cardiovascular-Kidney-Metabolic (CKM) Syndrome receives optimal medication management through team-based care including a pharmacist to ensure their medications are safe, effective, affordable and can be taken as intended.

OUR GOALS

 Foster learning and action to ensure best practices in medication management for patients with CKM syndrome. 

 Advance the inclusion of pharmacists on interdisciplinary care teams to bridge the gap between Nephrology, Cardiology, Endocrinology and Primary care for patients with CKM syndrome by implementing *CMM services. 

Demonstrate value of the pharmacist within value-based payment and fee-for-service models.
