OUR GOALS

Advance the inclusion of pharmacists on interdisciplinary care teams to bridge the gap between Nephrology, Cardiology, Endocrinology and Primary care for patients with Cardiovascular- Kidney-Metabolic (CKM) syndrome by implementing Comprehensive Medication Management (CMM) services.

 

Help meet new value-based payment model metrics and serve as evidence generating incubators for interdisciplinary CKM care.

 

Foster learning and action to ensure best practices in the care of patients with chronic kidney disease and CKM.
